ABSTRACT

Objective To analyze the clinical characteristics and risk factors of ( acute myocardial infarction , AMI ) in elderly patients with ( hyperuricemia , HUA) in Suzhou, and to provide a theoretical basis for the prevention and treatment of AMI in patients with HUA. Methods Among of 356 patients with hyperuricemia aged ≥60 years who were admitted to Suzhou Jiulong Hospital from January 2016 to December 2021 were selected. The patients were divided into control group and AMI group according to whether they were complicated with AMI. The clinical data of the patients were collected from the medical record system, and the types of AMI and the lesions of the three main coronary vessels in patients with HUA were statistically analyzed. The serum BNP level and LVEF value of the two groups were compared. Univariate analysis and logistic regression analysis were conducted to analyze the risk factors of AMI in patients with HUA. Results Fifty-five cases (15.45%) of 356 patients with HUA developed AMI, including 31 males and 24 females. There were 26 patients (47.27%) with NSTEMI and 29 patients (52.73%) with STEMI. In addition, 30 cases (54.55%) had three-vessel lesions. Compared with the control group, the male ratio, average age, smoking history ratio, systolic blood pressure, FPG, LDL-C and Hcy levels in the AMI group were significantly increased (P<0.05), while LVEF values decreased (P<0.05). Smoking history (OR=2.987), high LDL-C (OR=3.439), high Hcy (OR=2.672) and high FPG (OR=3.874) were risk factors for AMI in patients with HUA (P<0.05). Conclusion HUA patients in Suzhou area have a high risk of AMI, and most of them are STEMI patients, with the characteristics of multi vessel branch lesions. For HUA patients with smoking history, poor control of Hcy, LDL-C and FPG, the risk of AMI is higher. Smoking cessation and control of LDL-C, Hcy and FPG levels should be actively adopted to reduce the risk of AMI.

ABSTRACT

In order to judge the future development trend of science and technology, plan ahead and lay out the frontier technology fields and directions, China Association of Chinese Medicine(CACM) has launched consultation projects for collecting "major scienti-fic issues and engineering technology difficulties in traditional Chinese medicine(TCM)" for the industry for three consecutive years since 2019. Up to now, 18 projects have been selected as major issues for research, and some experience and achievements have been made. These projects have been applied in important scientific and technological work such as scientific and technological planning and deployment at all levels of national, local, and scientific research institutions, the selection and cultivation of major national scientific and technological projects, and the construction of innovation bases, giving full play to the role of the think tank advisory committee of CACM. This study reviewed the selection of major issues for the first time, systematically combed its application in the national layout of science and technology, and put forward the existing problems and improvement suggestions, aiming to provide new ideas for further improving the selection of major issues and research direction, providing a theoretical basis and decision support for the national scientific and technological layout in the field of TCM, and promoting scientific and technological innovation to facilitate the high quality development of TCM.

ABSTRACT

Children's fever is often accompanied by food accumulation. Traditional Chinese medicine believes that removing food stagnation while clearing heat of children can effectively avoid heat damage. To systematically evaluate the efficacy of Xiaoer Chiqiao Qingre Granules(XRCQ) in clearing heat and removing food accumulation and explore its potential mechanism, this study combined suckling SD rats fed with high-sugar and high-fat diet with injection of carrageenan to induce rat model of fever and food accumulation. This study provided references for the study on the pharmacodynamics and mechanism of XRCQ. The results showed that XRCQ effectively reduced the rectal temperature of suckling rats, improved the inflammatory environment such as the content of interleukin-1β(IL-1β), interleukin-2(IL-2), interferon-γ(IFN-γ), white blood cells, and monocytes. XRCQ also effectively repaired intestinal injury and enhanced intestinal propulsion function. According to the confirmation of its efficacy of clearing heat, the thermolytic mechanism of XRCQ was further explored by non-targeted and targeted metabolomics methods based on LTQ-Orbitrap MS/MS and UPLC-QQQ-MS/MS. Non-target metabolomics analysis of brain tissue samples was performed by QI software combined with SIMCA-P software, and 22 endogenous metabolites that could be significantly regulated were screened out. MetaboAnalyst pathway enrichment results showed that the intervention mechanism was mainly focused on tyrosine metabolism, tricarboxylic acid cycle, inositol phosphate metabolism, and other pathways. At the same time, the results of targeted metabolomics of brain tissue samples showed that XRCQ changed the vitality of digestive system, and inhibited abnormal energy metabolism and inflammatory response, playing a role in clearing heat and removing food stagnation from multiple levels.

ABSTRACT

ObjectiveTo explore the intervention effect and mechanism of Buzhong Yiqiwan (BZYQ) on colitis mice. MethodSixty-four C57BL/6 mice were randomly divided into 2 weeks blank group, 2 weeks model group, 2 weeks high-dose BZYQ (12 g·kg-1) group, 2 weeks low-dose BZYQ (6 g·kg-1) group, 4 weeks blank group, 4 weeks model group, 4 weeks high-dose BZYQ (12 g·kg-1) group, and 4 weeks low-dose BZYQ (6 g·kg-1) group. The colitis model was induced in mice by feeding 3% dextran sodium sulfate (DSS) for 7 days. The mice received BZYQ (12 and 6 g·kg-1) by gavage on the 8th day after modeling, once per day, and sacrificed on the 2nd and 4th weeks, correspondingly. The colon length and weight of mice in each group were measured. Hematoxylin-eosin (HE) staining was used for pathological observation and colonic mucosal inflammation was scored. The mRNA and protein expression of NOD-like receptor thermoprotein domain 3 (NLRP3), apoptosis-associated speck-like protein containing a CARD (ASC), and cysteinyl aspartate-specific protease-1 (Caspase-1) was detected by real-time quantitative polymerase chain reaction (Real-time PCR) and Western blot, respectively. Enzyme-linked immunosorbent assay (ELISA) was used to detect the content of inflammatory cytokines, such as interleukin (IL)-1β, IL-18, and IL-33 in colonic tissues. ResultCompared with the 2 weeks blank group, the 2 weeks model group showed shortened colon length, increased colon weight (P<0.05), loss of epithelial cells, destruction of gland structure, infiltration of a large number of inflammatory cells in mucosa and submucosa, local crypt abscess, and increase in mucosal inflammation score (P<0.01) as revealed by light microscopy, elevated levels of IL-1β, IL-18, and IL-33 in colonic tissues (P<0.05), and increased mRNA and protein expression of NLRP3, ASC, and Caspase-1 (P<0.05). The intervention of BZYQ (12 g·kg-1) restored colon length, alleviated mucosal injury (P<0.05), down-regulated the content of IL-18 (P<0.05), reduced the mRNA expression of NLRP3 and ASC as well as the protein expression of ASC and Caspase-1 compared with the conditions in the 2 weeks model group. Compared with the 4 weeks blank group, the 4 weeks model group showed decreased colon length, increased colon weight (P<0.05), decreased glands in the mucosal layer, expansion of glandular cavity, atrophy of crypt, local connective tissue hyperplasia and lymphocyte infiltration, increased inflammation score (P<0.01) as revealed by the light microscopy, increased expression of IL-1β, IL-18, and IL-33 (P<0.05), and elevated mRNA and expression of NLRP3 inflammasome complex (P<0.05). Compared with the conditions in the 4 weeks model group, the intervention of BZYQ (12 and 6 g·kg-1) could improve colon length and weight (P<0.05), and the intervention of BZYQ (12 g·kg-1) could also improve the inflammation score of the colon (P<0.05). Different from the acute stage, the intervention of BZYQ (12 and 6 g·kg-1) increased the content of IL-33 in the intestinal mucosa and up-regulated the mRNA and protein expression of NLRP3 inflammasome complexes ASC and Caspase-1 (P<0.05). ConclusionBZYQ can relieve the injury of colitis induced by DSS in mice. The mechanism is related to the regulation of intestinal immune response mediated by NLRP3 inflammasome, and it has different regulatory effects in acute and chronic inflammation stages.

ABSTRACT

By referring to the materials of ancient materia medica, prescriptions and medical books, combining with modern literature, this paper made textual research on the name, origin, producing area, quality evaluation, harvesting and processing methods of Ligustici Rhizoma et Radix in famous classical formulas, so as to provide a basis for the selection and use of Ligustici Rhizoma et Radix in the development of famous classical formulas. Through textual research, it can be seen that Ligustici Rhizoma et Radix is the correct name in all dynasties, but it still has many aliases. The main strain of Ligustici Rhizoma et Radix was Ligusticum sinense and L. jeholense, in addition, there are also L. tenuissimum, Sinodielsia yunnanensis, and Conioselinum vaginatum, L. sinense var. hupehense used in different regions. Since modern times, the textual examination of its scientific name is more complicated. Different foreign scholars have given many scientific names from their local observation, but most of them do not match the actual situation of Ligustici Rhizoma et Radix in ancient China. It is probably because they did not collect the original plants of Chinese Ligustici Rhizoma et Radix and conduct accurate identification. After the founding of the People's Republic of China, the L. sinense and L. jeholense was identified as the original plants through systematic investigation and arrangement of Ligustici Rhizoma et Radix. Since modern times, L. sinense is distributed in the upper and middle reaches of the Yellow River and the south of the region, a small amount of production in Sichuan, Hubei, Hunan and Jiangxi. L. jeholense is distributed in the middle and lower reaches of the Yellow River and the north of the region, mostly produced in Liaoning and Hebei. The ancient harvesting period of Ligustici Rhizoma et Radix was concentrated in January and February according to the lunar calendar with thick roots, dry, fragrant, heavy quality as the best. But now the collection of roots and rhizome more than concentrated in spring before seedling emergence and autumn when leaves were gone, and take the large, dry, yellow, strong, fragrant, less roots, residual short as the best. In the past dynasties, raw products were the main processing methods, and there were also roasting, grinding and so on. Based on the research results, it is suggested that the raw roots and rhizomes of L. sinense and L. jeholense should be used in the development of famous classical formulas.

ABSTRACT

The development of traditional Chinese medicine(TCM) has always been highly valued and supported since 1949. However, Chinese medicine industry still faces great challenges in view of the current status of the industry and registration and approval of new products in recent years. Related policies also directly influence the development of the industry. The latest version of the Provisions for Drug Registration and Requirement on Registration Classification and Application Information of Traditional Chinese Medicines have been put into practice since 2020. Registration classification is the core content of the Chinese medicine registration management system, as it is closely related to the research, development, and registration of Chinese medicine and the innovative development of the industry. This article aims to systematically review the historical evolution of the category of Chinese medicine registration and analyze the current status and problems, which is expected to provide a reference for the formulation of supporting documents according to related laws and regulations.

ABSTRACT

Objective:To investigate the changes of serum sex hormones, 25-hydroxyvitamin D3, 25 (OH) D3] levels and islet function in patients with gestational diabetes mdlitus (GDM) , and to analyze their intrinsic relationships.Methods:50 GDM patients admitted to the Department of Clinical Medicine of Wuhan Traditional Chinese Medicine Hospital from Mar. 2018 to Sep. 2019 were selected as the study group, and 50 healthy pregnant women were selected as the control group. Serum level of sex hormones, 25 (OH) D3 level, islet function [Fasting insulin (FINS) , Insulin resistance index (HOMA-IR) , Islet β cell function index (HOMA-β) ], the correlations between serum sex hormones, 25 (OH) D3 levels, and islet function changes were analyzed using Person, and the influencing factors of GDM were analyzed using binary logistic.Results:The estrogen level, progesterone level, FINS level and HOMA-IR of the observation group were significantly higher than those of the control group[ (6525.28±2095.51) vs (2259.02±717.75) pg/ml, (554.34±32.85) vs (385.34±24.59) ng/ml, (12.69±3.93) vs (9.68±3.19) mU/L, (3.06±1.06) vs (2.01±0.63) ]. 25 (OH) D3 and HOMA-β in the observation group were significantly lower than those in the control group[ (17.46±5.59) vs (21.51±7.14) ng/ml, (137.31±32.11) vs (281.76±54.02) ], the differences were statistically significant ( P<0.05) . According to Pearson analysis, sex hormones were positively correlated with FINS and HOMA-IR ( P<0.05) and negatively correlated with HOMA-β ( P<0.05) ; 25 (OH) D3 was positively correlated with HOMA-β ( P<0.05) and progestin was negatively correlated ( P<0.05) ; FINS was positively correlated with HOMA-IR ( P<0.05) , and HOMA-IR was negatively correlated with HOMA-β ( P<0.05) . 25 (OH) D3 and HOMA-β were risk factors for GDM ( P<0.05) , and FINS and HOMA-IR were protective factors for GDM ( P<0.05) . Conclusions:GDM patients have higher levels of sex hormones, lower levels of25 (OH) D3, HOMA-β, vitamin D deficiency or deficiency, and decreased insulin secretion. There is a correlation between 25 (OH) D3 and HOMA-β.

ABSTRACT

It is the core of the development for Chinese patent medicine enterprises to cultivate large varieties of Chinese patent medicine, and the selection of potential "seed" products is the prerequisite for the cultivation strategy. By constructing the evaluation model from multiple dimensions of value and risk, we can conduct specialized evaluation of Chinese patent medicines to effectively, professionally and objectively select the "seed" products with large variety cultivation potential. In this paper, the establishment of a multidimensional evaluation system would be discussed from the aspects of drug naming and prescription composition, safety risk and supply guarantee of raw materials and medicinal materials, competition situation, access to policy catalogue, scientific and technological support, clinical evidence and recognition, systematical and standardized collection of information on product instructions, quality standards, policy catalogue, scientific and technological literature, market competition and clinical application of Chinese patent medicines. Through the objective evaluation index and the range of objective index, the multi-dimensional evaluation model on values and risks of Chinese patent medicine products was discussed. Based on this model, a batch of Chinese patent medicine products can be quickly and comprehensively analyzed, and quantitative comparison can be formed among different types and fields of products. According to the evaluation results of the model and the comprehensive evaluation of experts, high-quality "seed" products can be selec-ted, laying a solid foundation for the next step of large variety cultivation. With use of this model, we can further clarify the external competitive advantages and internal priority levels of each product, and provide support for enterprises to optimize product structure and improve product strategic layout.

ABSTRACT

Objective:To investigate the molecular mechanism of Qiyu Sanlong prescription (QYSL) in inhibiting the "addiction" of lung cancer A549 cells to miRNA21. Method:The human lung cancer A549 cells were routinely passaged and divided into the blank group, blank serum group, QYSL-containing serum group, and siRNA group. The prepared QYSL-containing serum was used for intervention, with the optimal concentration and action time determined in previous studies. The protein and mRNA expression levels of miRNA21 and related molecules in its target phosphatase and tensin homolog deleted on chromosome 10 (PTEN)/phosphatidylinositol 3-kinase (PI3K) signaling pathway were detected by real-time polymerase chain reaction (Real-time PCR) and Western blot assay. Result:The comparison with the blank serum group revealed that the mRNA expression levels of miRNA21 in the QYSL-containing serum group and the siRNA group were decreased, while the PTEN mRNA expression in the QYSL-containing serum group was increased, showing significant differences (<italic>P</italic><0.01). Compared with the blank serum, the QYSL-containing serum and siRNA significantly down-regulated PI3K and mammalian target of rapamycin (mTOR) mRNA expression (<italic>P</italic><0.01), whereas the QYSL-containing serum did not change the mRNA expression of protein kinase B (Akt). The protein expression levels of PTEN in the QYSL-containing serum group and the siRNA group were obviously elevated in contrast to that in the blank serum group (<italic>P</italic><0.05). Meanwhile, the protein expression levels of phosphorylated Akt (p-Akt) and phosphorylated mTOR (p-mTOR) evidently declined in the QYSL-containing serum group (<italic>P</italic><0.05), but there was no significant reduction in total Akt and mTOR protein expression. The PI3K protein expression was slightly down-regulated, with no statistical significance. Conclusion:QYSL inhibits the transcription of miRNA21, increases the expression of PTEN, and reduces the expression of key molecules in PI3K/Akt/mTOR signaling pathway, thus mildly inhibiting the "addiction" of lung cancer cells to oncogenes and blocking their proliferation.

In recent years, the incidence of neurological diseases has been increasing year by year. To give full play to the advantages of traditional Chinese medicine (TCM) in the treatment of neurological disorders, identify the breakthrough point of integrating TCM with western medicine, and further standardize the clinical diagnosis and treatment of TCM, the China Association of Chinese Medicine organized neurologists in TCM and western medicine to carry out in-depth discussion on the neurological diseases responding specifically to TCM and integrated TCM and western medicine, such as stroke, headache, vertigo, multiple sclerosis, and epilepsy, aiming to formulate a well-recognized and integrated treatment protocol for TCM and western medicine and improve the efficacy of neurological disorders. Furthermore, the treatment suggestions of the corresponding diseases in TCM and western medicine were proposed to provide references for clinical practice and scientific research.

ABSTRACT

Objective:To evaluate the effect of Mashao Pingchuan decoction on traditional Chinese medicine <bold>(</bold>TCM<bold>)</bold> symptoms, quality of life, peripheral blood eosinophils (Eos) and serum inflammatory factors interleukin-6 (IL-6) and interleukin-8 (IL-8) in patients with cold asthma syndrome of bronchial asthma. Method:A total of 67 patients with cold asthma who attended the Respiratory Clinic of Anhui Provincial Hospital of TCM from January 2018 to December 2019 were selected and randomly divided into a control group and an observation group. The control group was given basic treatments such as budesonide formoterol powder inhalation, and the observation group was given Mashao Pingchuan decoction on the basis of the control group. Both groups intervened for 7 consecutive days. Observe and record the general condition, TCM symptom score, asthma control questionnaire (ACQ)-7 score, Marks-asthma quality of life questionnaire (Marks-AQLQ) score of the two groups of asthma patients, the peripheral blood Eos count was measured by hematology analyzer, and the serum IL-6 and IL-8 levels of the subjects were measured by enzyme-linked immunosorbent assay (ELISA). Result:The total effective rate of TCM symptoms in the two groups was 100%(30/30), and the effect in the observation group was more obvious (<italic>Z</italic>=-2.169,<italic>P</italic><0.05<italic>)</italic>. After treatment, the scores of TCM symptoms in the two groups were significantly lower than those before treatment (<italic>P</italic><0.01). The scores of phlegm in the throat, expectoration, cough, and chills in the observation group were lower than those in the control group (<italic>P</italic><0.05). After treatment, the ACQ-7 scores and Marks-AQLQ scores of the two groups were significantly lower than before treatment (<italic>P</italic><0.01). There was no statistically significant difference between the observation group and the ACQ-7 scores and Marks-AQLQ scores. After treatment, the peripheral blood Eos counts and serum IL-6 and IL-8 levels in the two groups were significantly lower than before treatment (<italic>P</italic><0.01). Peripheral blood Eos count and serum IL-6 and IL-8 contents in observation group were lower than those in control group (<italic>P</italic><0.05). Conclusion:Mashao Pingchuan decoction combined with budesonide formoterol powder inhalation can effectively improve the clinical effectiveness of asthma (cold asthma), improve the symptoms of asthma in TCM, ACQ-7 score, Marks-AQLQ score, peripheral blood Eos count, serum inflammatory factor content.

ABSTRACT

Objective:To investigate the effect of Jianpi Bufei prescription (JPBFP) on airway inflammation, airway hyperresponsiveness (AHR), and cyclic adenosine monophosphate (cAMP) signaling pathway activity in ovalbumin (OVA)-sensitized and challenged juvenile asthma rats. Method:Seventy-five male SD rats were randomly divided into a blank group (<italic>n</italic>=15) and an experimental group (<italic>n</italic>=60). The rats in the experimental group were sensitized by aluminum hydroxide gel containing 0.2% OVA and stimulated by aerosol inhalation of normal saline containing 1% OVA to induce an asthma model, followed by assignment into the following groups: a model group (<italic>n</italic>=15), a JPBFP group (<italic>n</italic>=15, 8.37 g·kg<sup>-1</sup>·d<sup>-1</sup>), an aminophylline group (<italic>n</italic>=15, 40 mg·kg<sup>-1</sup>·d<sup>-1</sup>), and a dexamethasone group (<italic>n</italic>=15, 0.1 mg·kg<sup>-1</sup>·d<sup>-1</sup>). AHR was detected by the pulmonary function analyzer, changes in inflammatory cells by white blood cell (WBC) count and differential blood count in bronchoalveolar lavage fluid (BALF), and pathological changes of lung tissues by hematoxylin-eosin (HE), Masson, and periodic acid-schiff (PAS) staining. The interleukin (IL)-4, IL-5, IL-13, interferon (IFN)-<italic>γ</italic>, and tumor necrosis factor (TNF)-<italic>α</italic> levels in serum and the cAMP level in plasma were tested by the enzyme-linked immunosorbent assay (ELISA). Protein kinase A (PKA) expression in lung tissues was detected by immunohistochemistry. The cAMP-response element-binding protein (CREB) mRNA and protein expression in lung tissues was detected by the real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) and Western blot. Result:Compared with the blank group, the model group showed increased lung resistance, decreased pulmonary compliance (<italic>P</italic><0.05), elevated WBC count and proportion of eosinophils in BALF (<italic>P</italic><0.05), up-regulated levels of IL-4, IL-5, IL-13, and TNF-<italic>α</italic> in peripheral blood, declining IFN-<italic>γ</italic> level (<italic>P</italic><0.01), severe pathological changes of lung tissues, dwindled cAMP, and down-regulated PKA and CREB expression (<italic>P</italic><0.01). Compared with the model group, JPBFP inhibited AHR, reduced WBC count and proportion of eosinophils in BALF and lung resistance (<italic>P</italic><0.05), improved pathological changes of lung tissues, increased pulmonary compliance, and up-regulated cAMP in serum and PKA and CREB expression in lung tissues (<italic>P</italic><0.01). Conclusion:JPBFP can improve AHR, inhibit airway inflammation, and alleviate lung injury in asthma rats. Its mechanism may be related to the up-regulation of the activity of the cAMP/PKA/CREB signaling pathway.

Objective:To seed for stable time window of the integrated disease-syndrome animal model based on the counterevidence from Chinese medicinal prescriptions, and to verify syndrome stability and reliability. Method:A model of depression was established by exposing rats to chronic unpredictable mild stress (CUMS), followed by body weight measurement, sugar water test, behavioral test, and brain 5-hydroxytryptamine(5-HT) detection. The identification of liver depression and spleen deficiency syndrome was conducted after the equivalent transformation of human clinical symptoms into macroscopic representations of rats. Based on the dynamically collected macroscopic representation scale, Xiaoyaosan was used to reversely verify the stability and reliability of the integrated disease-syndrome animal model of depression due to liver depression and spleen deficiency. Result:The sugar water consumption and the number of crossings and the total movement distance in the open field test of 16-week-old rats in the CUMS (eight weeks of CUMS) group were significantly lower than those in the normal group (<italic>P</italic><0.05). According to the immunohistochemical results, the 5-HT content in hippocampal area CA2 of rats in the CUMS group was also significantly lowered as compared with that in the normal group(<italic>P</italic><0.05),which indicated that depression was successfully modeled. The liver depression and spleen deficiency syndrome was present in 14-week-old rats (six weeks after CUMS)of the CUMS group, and the number of rats experiencing the liver depression and spleen deficiency syndrome reached the peak in the 16th week (eight weeks after CUMS),accounting for 70% of the total number. Thereafter, the number decreased gradually. The syndrome scores of the 14-, 16-, 18-, 20-, and 22-week-old rats in the Xiaoyaosan group were reduced by 66.6%, 70.7%, 54.8%, 50.4%, and 44.8%, which were graded as effective, marked effective, effective, effective, and effective, respectively. Conclusion:The age of 14-16 weeks(six to eight weeks after CUMS) is considered the stable and reliable time window for depression due to liver depression and spleen deficiency.

ABSTRACT

Objective:To observe the effect of Qiyu Sanlong decoction (QYSL) on the expressions of key molecules in signal axis of mammalian rapamycin target protein (mTOR)/yeast Atg6 homologous (Beclin1)/ microtubule-associated protein1 light chain3 (LC3) in A549 cells. Method:With A549 cells as the research object, the effect of QYSL medicated serum on cell viability of A549 cells were detected by cell counting kit-8 (CCK-8) method. The effect of QYSL decoction on A549 cell apoptosis, autophagosome formation and the expression of autophagy markers were detected by Terminal-deoxynucleoitidyl transferase mediated nick end labeling (TUNEL) method, transmission electron microscope (TEM), Real-time polymerase chain reaction (Real-time PCR) and Western blot. Result:QYSL medicated serum could inhibit the viability of A549 cells in a concentration-dependent manner. Compared with the blank serum group, the number of apoptotic A549 cells in the QYSL medicated serum group was significantly increased (P<0.01), and the formation of autophagosome was significantly increased. Compared with the blank serum group, the mRNA and protein expressions of mTOR in A549 cells in the QYSL serum group were significantly decreased (P<0.01), while mRNA and protein expressions of Beclin-1, autophagy related genes 5 (ATG5), autophagy related genes 13 (ATG13) were significantly increased (P<0.01). Conclusion:QYSL decoction can induce autophagy in A549 cells, and its specific mechanism may be related to the down-regulation of mTOR expression, the up-regulation of Beclin1, ATG5, ATG13 and LC3 expression, and the promotion of LC3Ⅰ conversion to LC3Ⅱ.

ABSTRACT

Objective::To study the effect of Qiyu Sanlong decoction on the growth of subcutaneous tumor in lung cancer mice and the expressions of key autophagy molecule, yeast Atg6 homologous (Beclin1), autophagy related genes5 (Atg5), and microtubule-associated protein1 light chain3 (LC3B). Method::Lewis lung carcinoma cells (LLC) were used to reproduce the lung cancer mice transplanted model. After the modeling, the mice were randomly divided into model group, Qiyu Sanlong decoction group, chemotherapy group and combination group, with 18 transplanted mice in each group. In model group, mice were fed with 0.9% saline 20 mL·kg-1 daily. In Qiyu Sanlong decoction group, mice were fed with Qiyu Sanlong decoction 80.48 g·kg-1 daily. The chemotherapy group was intraperitoneally injected with 0.4 mL cisplatin solution (DDP) at the 1st, 3rd and 5th day. The combination group was orally given the drugs at the concentration of 80.48 g·kg-1, and 0.4 mL DDP solution was intraperitoneally injected at the 1st, 3rd and 5th day. After 21 days of continuous treatment, tumor tissue was exfoliated and weighed, and the tumor inhibition rate was calculated. Hematoxylin-eosin (HE) staining was used to observe the histological changes of tumor. The expressions and localizations of Beclin1 and LC3B in tumor tissues were detected by immunohistochemical staining. Protein expressions of Beclin1, Atg5, LC3B-Ⅰand LC3B-Ⅱ were determined by Western blot, and the ratio of LC3B-Ⅱ/LC3B-Ⅰ was calculated. The transcription levels of Beclin1, Atg5 mRNA in tumor tissues were detected by Real-time PCR. Result::Qiyu Sanlong decoction had a mild inhibitory effect on transplanted tumor, with an inhibitory rate of 31.2%. Under microscope, patchy necrotic tumor cells were observed in the tumor tissues of Qiyu Sanlong decoction group. Immunohistochemical staining and Western blot analysis showed that Qiyu Sanlong decoction could up-regulate the expressions of Beclin1, Atg5 and LC3B protein (P<0.01), and promote the conversion from LC3B-Ⅰ into LC3-Ⅱ compared with the model group. Real-time PCR results showed that Qiyu Sanlong decoction could promote the transcription of Beclin1 mRNA and Atg5 mRNA compared with the model group (P<0.01). Conclusion::Qiyu Sanlong decoction has a mild inhibitory effect on lung tumors, and its mechanism may be related to up-regulating the expressions of autophagy key proteins Beclin1, Atg5 and LC3B, and promoting the conversion from LC3B-Ⅰ to LC3B-Ⅱ.

ABSTRACT

In recent years, high-sensitivity cardiac troponin T (hs-cTnT) has been recognized as an effective marker for myocardial injury in adults and can be used to diagnose acute myocardial injury and predict major adverse cardiovascular events. It is the gold standard for the diagnosis of acute myocardial infarction in adults. Neonates are a special group, and due to the changes of various physiological processes during the perinatal period, many laboratory markers used in adults may have a low clinical value in neonates. So far, for example, there is still no suitable cardiac serum biomarker that can reflect the true condition of neonatal myocardial injury. In recent years, new breakthroughs have been made in the application of hs-cTnT in the field of neonates. In order to fully understand the role of hs-cTnT in neonatal diseases, this article reviews the research advances in the biological and physiological features of hs-cTnT and its application in neonates.

BACKGROUND@#Positive surgical margins are independent risk factor for biochemical recurrence, local recurrence, and distant metastasis after radical prostatectomy. However, limited predictive tools are available. This study aimed to develop and validate a preoperative nomogram for predicting positive surgical margins after laparoscopic radical prostatectomy (LRP).@*METHODS@#From January 2010 to March 2016, a total of 418 patients who underwent LRP without receiving neoadjuvant therapy at Peking University Third Hospital were retrospectively involved in this study. Clinical and pathological results of each patient were collected for further analysis. Univariable and multivariable logistic regression (backward stepwise method) were used for the nomogram development. The concordance index (CI), calibration curve analysis and decision curve analysis were used to evaluate the performance of our model.@*RESULTS@#Of 418 patients involved in this study, 142 patients (34.0%) had a positive surgical margin on final pathology. Based on the backward selection, four variables were included in the final multivariable regression model, including the percentage of positive cores in preoperative biopsy, clinical stage, free prostate specific antigen (fPSA)/total PSA (tPSA), and age. A nomogram was developed using these four variables. The concordance index (C-index) of the nomogram was 0.722 in the development cohort and 0.700 in the bootstrap validations. The bias-corrected calibration plot showed a limited departure from the ideal line with a mean absolute error of 2.0%. In decision curve analyses, the nomogram showed net benefits in the range from 0.2 to 0.7.@*CONCLUSION@#A nomogram to predict positive surgical margins after LRP was developed and validated, which could help urologists plan surgical procedures.

ABSTRACT

Objective:To summarize medication law of prescription and clinical experience on the treatment of chronic cough through data mining technology. Method:The formula information of 650 cases of chronic cough cases diagnosed and treated by professor HAN Ming-xiang was processed with data mining technology, and system cluster analysis and factor analysis were conducted by SPSS Statistics 22.0. Result:High-frequency medicines of professor HAN Ming-xiang for treating chronic cough were Stemonae Radix, Citri Reticulatae Pericarpium, Asteris Radix et Rhizoma and other 37 herbs, their frequency of occurrence accounted for 80.52%. According to the efficacy, the medicines could be classified into 10 categories, such as expectorant cough antiasthmatic drugs, tonifying deficiency drugs, and diaphoretic drugs. Cluster analysis showed 5 combinations of drugs. Factor analysis yielded 13 common factors, such as Stemonae Radix-Asteris Radix et Rhizoma-Farfarae Flos-Cynanchi Stauntonii Rhizoma et Radix, Asari Radix et Rhizoma-Schisandrae Chinensis Fructus-Zingiberis Rhizoma-Fritillariae Thunbergii Bulbus. Conclusion:Combined with clinical experience, it can be concluded that clinical treatment of professor HAN Ming-xiang for chronic cough attaches great importance to the pathogenesis of wind-evil, Yang deficiency and cold phlegm underlying lung, phlegm and blood stasis and Qi stagnation. The treatment mainly starts from the lung and spleen, with the method of warm and moist, or with some traditional Chinese medicines with the function of removing phlegm by warming the lung, dispersing cold by thinning the surface, strengthening the spleen and resolving phlegm, strengthening the lung and fixing the surface, and relieving spasmolysis by wind.

ABSTRACT

Objective: To study and create the algorithm for the diversity (AD) of extensible markup language (XML) tree map, and provide a new tool for the identification of Artemisiae Annuae Herba. Method: According to the literature research, the key information of Artemisiae Annuae Herba was selected from the macroscopic, mesoscopic and microscopic information, etc. Based on the key information, the relevant upper standards of domestic and foreign professional fields were cited to assign the unique identification independent of language for each data element, and the coding rules of relevant data elements were established. The digital coding technology was applied to the flexible structure editor, and the tree map was created, which could be returned as the 5th version of hypertext markup language (HTML5) or XML format. Based on the diversity related algorithms, the authors innovatively developed the AD of XML tree map of Artemisiae Annuae Herba, which took into account both of topology and semantics, and the expression model of related mathematical functions of Artemisiae Annuae Herba was established. By comparing the calculation results with the reality, the algorithm model was debugged continuously until the convergence of the core-culvert algorithm model. Result: Through the research on AD, the diversity between two XML tree maps could be calculated, and the discrimination or identification model of Artemisiae Annuae Herba also could be finally optimized and established. After calculation and analysis of the tested tree maps, the effective rate of the model was 100%. Conclusion: In this study, the establishment of the AD of XML tree map can effectively assist in the identification of Artemisiae Annuae Herba, which provides certain technical support and theoretical guidance for the research on intelligent application of traditional Chinese medicine.

ABSTRACT

OBJECTIVE@#To evaluate the effectiveness and safety of Chinese herbs in the treatment of sub-health systematically.@*METHODS@#Nine databases were systematically and extensively searched to collect randomized controlled trials (RCTs) about Chinese herbs in the treatment of sub-health. The outcomes included overall effective rate, main symptoms, quality of life, etc. Literature screening, data extraction and quality assessment were conducted according to Cochrane Handbook 5.1. Meta-analysis was conducted to the included literature with Review Manager Software.@*RESULTS@#Seventy-two studies involving 9,296 patients with sub-health were included with 4,908 patients in experimental groups and 4,387 patients in control groups. The overall quality of included clinical research was not high. In the aspect of improving overall effective rate, relieving main symptoms, Pittsburgh sleep quality index (PSQI) score, Athens Insomnia Scale (AIS) score, Fatigue Scale-14 (FS-14), Cornell Medical Index (CMI) score and discontinuation rate, the effects of experimental groups were better than that of control groups. According to available research reports, adverse reactions in Chinese herb groups were mainly mild gastrointestinal symptoms, which did not affect the treatment.@*CONCLUSION@#Chinese herbs have a curative effect in the treatment of sub-health. However, there are no clear criteria for diagnosis and curative effectiveness judgment globally, which would affect the accuracy of curative effect evaluation.
